After lockout is completed, the lockout authority will purge and/or “Test” the equipment or system to verify that it is de-energized while locked out.
Appropriate PPE and safety measures shall be used during test operations.
The responsible employee(s) will install their daily “Equipment Locked-Out By” tags and locks on
the lockout device prior to commencing work.
These daily “Equipment Locked-Out By” tags and locks will be logged in the tag/lockout log book.
Information to be noted on daily “Equipment Locked-Out By” tag shall include the equipment locked-out, date tag installed, owners name, printed and signed, craft and brass number.

The supervisor/foreman is responsible for ensuring that his workers install their daily tags and

When there are no provisions for locking out, the equipment will be tagged-out and isolated by
removing valves, pipe spool pieces, or disconnecting key connections.
